About the Role

As a Home Health Occupational Therapist at CenterWell, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. You will deliver personalized care that focuses on rehabilitation and functional improvement, empowering individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.

In this position, you will assess patients' daily living and work-related skills, develop therapeutic retraining programs with measurable objectives, and administer diagnostic tests to assist physicians in evaluating functional levels. You will collaborate with physicians and the interdisciplinary team to develop and revise Plans of Care, providing therapeutic treatment to improve strength, coordination, and function. Your day will involve consulting with vocational team members, designing community reintegration activities, and recommending adaptive equipment to improve clients' living or working environments. Additionally, you will supervise and instruct Occupational Therapy Assistants and Home Health Aides, while ensuring accurate documentation of all care observations and interventions.
