As a Home Health Occupational Therapist, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.
You will assess patients' daily living and work-related skills, develop therapeutic retraining programs with measurable objectives, and administer diagnostic tests to assist physicians in evaluating functional levels. Your day will involve conferring with physicians and the healthcare team to develop and revise Plans of Care, providing therapeutic treatment to improve strength and function, and consulting with vocational team members to implement vocationally oriented plans. You will also design community reintegration activities, recommend adaptive equipment, and train staff and family members on retraining programs. Additionally, you will supervise Occupational Therapy Assistants and Home Health Aides, ensuring accurate documentation and participating in care coordination and discharge planning.
This role is part of Humana's Tuberculosis (TB) screening program and driver safety program. If selected, you will be required to be screened for TB and must maintain a valid state driver's license and personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
