Use your skills to make an impact

In this role, you will partner with physicians and the interdisciplinary team to develop and revise Plans of Care. You will assess patients' daily living and work-related skills, administer diagnostic tests, and design therapeutic retraining programs with measurable objectives. Your work will involve consulting with vocational team members to create vocationally oriented plans, designing community reintegration activities, and recommending adaptive equipment to improve clients' environments. You will also train staff and family members on retraining programs and coordinate care transitions and discharge planning.
