As a Home Health Occupational Therapist, you will partner with patients to assess their daily living and work-related skills, developing therapeutic retraining programs with measurable objectives. Your day will involve administering diagnostic tests to evaluate function, collaborating with physicians and the healthcare team to refine Plans of Care, and providing therapeutic treatment to restore strength, coordination, and range of motion.
You will serve as a resource for patients and families, designing community reintegration activities and recommending adaptive equipment to improve living environments. A key part of your role involves coordinating care, overseeing Occupational Therapy Assistants and Home Health Aides, and ensuring thorough documentation of all patient observations and interventions. You will also train staff and family members on retraining programs, from dressing and feeding to hygiene skills, ensuring a holistic approach to patient recovery.
