About the Role

As a Home Health Occupational Therapist, you will play a vital role in helping patients regain strength, mobility, and independence from the comfort of their homes. You will deliver personalized care focused on rehabilitation and functional improvement, empowering individuals to overcome physical limitations and perform everyday activities with confidence.

Your day will involve assessing patients' daily living and work-related skills to develop therapeutic retraining programs with measurable objectives. You will administer diagnostic tests to assist physicians in evaluating functional levels and collaborate with the patient's physician and the broader healthcare team to develop and revise Plans of Care. You will provide therapeutic treatment to improve strength, coordination, and range of motion, while also consulting with vocational team members to implement vocationally oriented plans. Additionally, you will design community reintegration activities, recommend adaptive equipment, and train staff and family members on retraining programs. You will also supervise Occupational Therapy Assistants and Home Health Aides, ensuring accurate documentation of all care observations and interventions.

Hiring Process

This role is patient-facing and part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. As this position involves driving to patient locations, it is part of Humana's driver safety program, requiring a valid state driver's license and personal vehicle liability insurance meeting state minimums or specific coverage limits.
