As an ACT Nurse (RN), you will join one of our Assertive Community Treatment (ACT) teams to provide compassionate, community-based care. This role combines clinical expertise with direct nursing care and support designed to help clients thrive in their own homes and communities.
ACT, ACT TAY (for Transitional-Aged Youth), and STEPS programs deliver wrap-around psychiatric services directly to people with chronic mental health disorders. Our multidisciplinary teams—comprising psychiatrists, social workers, nurses, substance use specialists, supported employment experts, and peer supporters—work together 24/7 to meet the diverse needs of those we serve. In this position, you will partner with clients and the team to develop individualized treatment plans, manage medications, and provide health education, ensuring every client receives the advocacy and care they deserve.
