This role exists to provide unmatched insight into a member's overall health and well-being that is often difficult to capture during routine office visits. During a visit lasting up to one hour, you will review and observe a member's current health, medical history, medication adherence, social environment, and other risks. The Matrix Comprehensive Health Assessment (CHA) allows you to improve quality of care and potentially close multiple care gaps with a single visit.
You will work directly at the member's place of residence, which may be their home or a Skilled Nursing Facility. This position offers the flexibility to schedule visits based on your availability, with assessments conducted Sunday through Saturday from 8 am to 7 pm. You can choose to work during the day, evening, and/or weekend to fit your lifestyle. The role involves traveling up to 80-90% of the time, including overnight travel and weekends, across your assigned state and possibly neighboring states.
To join our team, you will undergo comprehensive onboarding training where you will be assigned a preceptor. We provide 1:1 feedback and offer free access to continuing education to support your growth. Should business needs exist, we will support you in obtaining additional state licensure and credentialing in neighboring states or other locations you may wish to visit.
