This role centers on the Matrix Comprehensive Health Assessment (CHA), a unique service that provides unmatched insight into a member's overall health and well-being—information often difficult to capture during routine office visits. During a visit lasting up to one hour, you will review and observe a member's current health, medical history, medication adherence, social environment, and other risks. This comprehensive approach allows us to potentially close multiple care gaps with a single visit, significantly improving the quality of care.
You will work directly at the member's place of residence, whether in their private home or a Skilled Nursing Facility. Your day will involve collaborating with Primary Care Physicians, case managers, social workers, family members, and key caregivers to ensure seamless patient education and follow-up. By conducting adult and geriatric assessments and administering point-of-care testing as needed, you will play a pivotal role in identifying chronic conditions that may otherwise go undiagnosed.
