Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role is designed for those with a passion for engaging with people and helping them on their journey to better health.
You will play a critical role in building trusted relationships to make people healthier, bringing flexible, comprehensive, and personalized health evaluations right to people's front doors. The visit lasts about 45 minutes, allowing time to connect one-on-one and answer health questions. You will conduct a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests. You'll use an iPad and an intuitive clinical workflow for each evaluation.
Clinicians in this role often highlight the unique ability to see health plan members in their own homes, gaining a better picture of their health context. The role allows time to truly listen to questions and concerns, guide patients on self-care, and provide education without the pressure of administrative burdens or rushing out the door.
