The Nurse Practitioner at Oak Street Health provides effective and equitable value-based primary care to adults on Medicare, with the goal of keeping them happy, healthy, and out of the hospital. You will see patients independently while collaborating with physicians based on specific patient needs. This role focuses on providing longitudinal care and building meaningful patient relationships that create trust and help patients feel connected to their care plans.
You will practice in our outpatient settings as part of a collaborative care team. This team typically includes a physician, nurse practitioners or physician assistants, medical assistants, a dedicated medical scribe, registered nurses, and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care, allowing our providers to focus on primary care, care coordination, and population health within the clinic.
