The Nurse Practitioner at Oak Street Health provides effective and equitable value-based primary care to adults on Medicare, with a mission to keep them happy, healthy, and out of the hospital. This role is designed for providers who want to practice independently while collaborating with physicians based on specific patient needs.
You will practice in an outpatient setting as part of a collaborative care team that includes physicians, physician assistants, medical assistants, a dedicated medical scribe, registered nurses, and care managers. This structure allows you to focus on primary care, care coordination, and population health within the clinic, while partnering with a network of elite specialists and hospitals for specialty and acute care needs.
In this role, you will build meaningful patient relationships that create trust and help patients understand and feel connected to their care plans. You will provide longitudinal care, conducting office visits for routine and acute issues, administering annual wellness visits, and leading population health initiatives. The position reports to the Center Medical Director and works closely with operational leadership.

Experience in primary care (internal medicine, geriatrics, or family medicine). Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs). Passion for teamwork and cross-functional collaboration. Desire to be part of an innovative model focused on empirically-guided population health. Bilingual proficiency in applicable areas.
