About the Role
Senior Community Care is a dedicated PACE (Program of All-Inclusive Care for the Elderly) provider committed to helping older adults remain safely independent within their communities. We are seeking a compassionate Nurse Practitioner to join our team and deliver primary and preventive care across home, clinic, and community-based settings.
This role is designed for a team-oriented clinician who values dignity, respect, and holistic care. You will serve as a vital partner to seniors, conducting comprehensive assessments, managing acute and chronic conditions, and developing individualized care plans that prioritize safety and quality of life. Your work will directly impact the well-being of our participants by coordinating seamless care transitions and reducing avoidable hospital readmissions.
In this position, you will collaborate closely with an interdisciplinary team of physicians, nurses, therapists, and social workers. Together, you will ensure that every participant receives coordinated, high-quality support that empowers them to live independently for as long as possible.
