Located at the UCHealth Anschutz Outpatient Pavilion in Aurora, CO, this full-time position within the Neurology department serves as a vital patient advocate, liaison, and educator. You will act as a bridge between patients, families, clinical staff, and external resources to navigate the continuum of care.
In this onsite role, you will regularly round on assigned patients to evaluate progress against their plan of care. Your day involves communicating care plans, soliciting concerns, and providing emotional support and clinical education to empower patients and families in their decision-making. You will coordinate resources, identify barriers, and assist with resolving financial, psycho-social, and administrative issues, requiring critical thinking and decisive judgment in a fast-paced environment.
