Located at the Mercy Clinic Springfield Gastroenterology Clinic, this position serves as a procedural nurse and Nurse Navigator. As a leader, you will provide support, mentorship, and guidance to those who deliver direct patient care. Your role is pivotal in coordinating the delivery of care by assessing individual patient requirements and making recommendations based on the severity of illness, intensity of service, and skill level.
You will ensure patients receive timely, safe, effective, and high-quality care by coordinating the patient's plan of care with the bedside nurse. This position is designed to facilitate an effective new care model, supporting quality patient care, safety, and financial components while promoting the integration of a seamless care model. You will hold direct and indirect care providers accountable for their professional responsibilities and adherence to regulatory guidelines.
Your day-to-day responsibilities include managing an assigned inpatient and outpatient population, coordinating patient discharge planning for post-acute services, managing length of stay, and optimizing performance metrics. You will lead pre-op education, conduct daily rounding while patients are in the hospital, determine patient discharge disposition, and oversee follow-up care. All duties are performed in a manner consistent with Mercy's mission, values, and service standards.
