Located at the UCHealth Anschutz Outpatient Pavilion in Aurora, CO, this full-time onsite position within the Hematology Clinic serves as a vital patient advocate, liaison, and educator. The role exists to assist patients in navigating the continuum of care, ensuring they can meaningfully participate in their healthcare process and personal decision-making.
In this capacity, you will act as a bridge between patients, families, clinical staff, administration, and external resources. Your day involves regularly rounding on assigned patients to evaluate progress against their plan of care, while providing emotional support, counseling, and expert guidance. You will identify and resolve barriers to care, assisting patients with financial, psycho-social, and administrative challenges by connecting them with appropriate resources. Success in this fast-paced environment requires critical thinking, decisive judgment, and the ability to work with minimal supervision.
