HSL provides enhanced housing with services to seniors living in its four affordable housing locations and at partner housing sites. The goal is to improve quality of life and support independent living through a proactive approach. Resident services staff regularly and actively reach out to each individual to engage around health and wellness, identify areas of need or risk, and provide intensive, individualized case management as desired by the resident.
The Nurse Care Manager is a key member of the housing team, working to support residents in living independently and safely for as long as possible. This role focuses on developing meaningful relationships with residents and providing holistic support. The position is based on-site at an affordable housing community in Chelsea.
A critical component of this role is the ability to connect with the community. The position requires fluency in Spanish, as the majority of the resident population is Spanish-speaking. The Nurse Care Manager collaborates with team members to engage residents in wellness assessments, health education, and care coordination, ensuring a supportive environment where residents feel seen and heard.
Hebrew SeniorLife employees set the highest standard in our commitment to our core values of respect, dignity, and empowerment. We foster a multicultural community where active outreach is essential. Staff are expected to be visible and connected, engaging with residents in their apartments, during programs, and meals. We value a "can-do" service mentality, where team members listen attentively, speak respectfully, maintain confidentiality, and actively promote inclusion for all residents and staff.
