HSL provides enhanced housing with services to seniors living in its four affordable housing locations and at partner housing sites. At its core, this approach is proactive: resident services staff regularly and actively reach out to each individual to engage around health and wellness, identify areas of need or risk, and provide intensive, individualized case management and support as desired by the resident.

Key Responsibilities
- check_circleConduct group and individual education sessions on health, wellness, and medication management
- check_circleIdentify and plan focused programs such as falls prevention using collected data
- check_circleAssist residents with transitions to other levels of care when necessary
- check_circleDocument all interventions and work electronically in shared files and computer programs
- check_circleConduct regular preventative outreach to assess resident needs and build trusting relationships
- check_circleManage follow-up on hospitalizations, rehab stays, and emergency visits to ensure safe discharges
- check_circleProvide comprehensive case management services to residents in partnership with the wellness team
- check_circleCoordinate with physicians, hospitals, and community providers to ensure effective communication
- check_circleAssess medical concerns and guide residents through decision-making for next steps in care
- check_circleEducate housing staff on medical conditions and methods to identify and report status changes
- check_circlePerform wellness assessments to identify risks and coordinate necessary referrals and resources
- check_circleSupport adherence to health plans, medications, and treatment protocols for at-risk residents
Requirements
- verifiedRN license with 1 year experience or LPN license with 2 years experience
- verified3 years experience in aging services
- verifiedSkills and comfort using Windows, Word, and Excel
Nice to Have
Home health experience, dementia care experience, experience in aging services in community, home health, or long-term care settings.
