The Mental Health Worker serves as a vital link for adult consumers in the community living with Severe and Persistent Mental Illness. This role is designed to provide comprehensive care management and coordination through a person-centered approach that incorporates cultural appropriateness. You will work directly with individuals and their families to develop Individualized Rehabilitation Plans, ensuring that care is tailored to unique needs.
In this position, you will facilitate linkages to essential services, offer supportive counseling, and provide crisis intervention. A key aspect of the role involves collaborating with family members and health care professionals to ensure seamless care transitions and the establishment of robust community supports. You will also assist with treatment enrollment and provide transportation as needed, ensuring that every consumer receives the holistic support required for their recovery journey.
