As a Medical Provider, you will deliver high-quality, patient-centered medical care as part of an integrated, multidisciplinary team. You will partner with individuals experiencing homelessness and other barriers to health, utilizing evidence-based, trauma-informed approaches to assess, diagnose, treat, and manage acute and chronic conditions.
Your day will involve working across multiple settings, including our clinic locations, mobile health units, and community-based sites. You will actively participate in care planning, team huddles, and multidisciplinary case conferences alongside medical, behavioral health, case management, and outreach staff. This role requires the flexibility to adapt to changing environments while maintaining a compassionate connection with clients who may be ill, distressed, or experiencing crises.
Both part-time and full-time schedules are available, with daytime hours offered four days a week. The position includes participation in a shared telephone on-call rotation.

Minimum of 3 years of direct clinical care experience (residency may satisfy this requirement for physicians). Experience in a community or public health setting working with underserved communities. Comfort working with individuals experiencing homelessness, behavioral health conditions, substance use disorders, and complex social needs.
