About the Role

As a Medical Case Manager I, you will become an integral part of a professional team that drives our company's mission and values forward. This role is designed for someone passionate about patient care who is ready to make a tangible difference every day. You will coordinate comprehensive case management services, ensuring clients have access to the care and support they need while adhering to local, state, and federal guidelines.

In this position, you will act as a vital liaison between patients and the broader care team. Your day will involve conducting eligibility screenings, developing individualized care plans, and linking clients to essential resources. You will collaborate with healthcare providers to promote optimal health outcomes, focusing on increasing patient functioning and self-sufficiency across all areas of life. Whether assisting with insurance verification, attending medical appointments, or documenting client encounters, your work directly supports the well-being of our community.

Key Responsibilities

  • check_circleScreen clients for program eligibility and conduct biannual reassessments
  • check_circleVerify insurance coverage and coordinate required pre-authorizations
  • check_circleDevelop and update individualized care plans based on patient needs
  • check_circlePerform comprehensive assessments to identify health and social support needs
  • check_circleCoordinate client access to primary medical care and treatment
  • check_circleSubmit timely and accurate monthly billing documentation in accordance with grant standards
  • check_circleDocument all client encounters within software applications within 48-72 hours
