As a member of the Patient Centered Health Care Home (PCHCH) team at Kalihi-Palama Health Center, you play an integral role in our mission to provide patient-centered health care. Our team is dedicated to ensuring every patient receives coordinated, high-quality care through seamless collaboration between providers and support staff.

About the Role

This position serves as a vital support to providers and the broader care team, managing the overall coordination, preparation, examination, and treatment of patients. You will be the face of the clinic during check-in and check-out, ensuring a smooth and welcoming experience for every visitor. Your day involves reviewing patient records, identifying gaps in care, and performing clinical procedures such as taking vital signs, administering medications, and assisting with wound care under the direction of a provider.

Beyond clinical duties, you will manage administrative tasks including scheduling, inventory management, and maintaining accurate documentation in the Electronic Medical Record (EMR). You will also serve as a communication hub, retrieving and returning calls from patients, staff, and community agencies, and following up on lab results. Success in this role requires a commitment to safety, a willingness to accept feedback for continuous improvement, and the ability to adapt to the dynamic needs of our diverse patient population.
