The Manager of Healthcare Partnerships serves as a strategic bridge between Mom's Meals and key stakeholders in the Northern California healthcare ecosystem. This role is dedicated to driving growth within the Managed Medicaid channel by cultivating deep, relationship-oriented partnerships with Managed Medicaid plans, Area Agencies on Aging (AAA), Community Based Organizations (CBOs), PACE programs, and state case management agencies.
At its core, this position focuses on supporting health plan and agency partners in successfully implementing Medically Tailored Meal programs. You will expand your knowledge of referral pathways by leading field-based members and ensuring that Mom's Meals is recognized as an industry leader in quality, customer service, and nutritional support for members with chronic health conditions. The role requires a proactive approach to client engagement, combining responsive account management with the ability to educate diverse audiences—including case managers, social workers, physicians, nurses, and dietitians—on the critical importance of medical nutrition in reducing healthcare costs for high-risk populations.

Non-credentialed candidates with passion and competency in consultatively networking and upselling SDOH benefits to field-based Medicaid case managers and their supervisors. Familiarity with CalAIM and Community Supports.
