About the Role
As an LPN Hospice Homecare Nurse, you will be a vital part of the interdisciplinary team supporting patients and families at critical junctures in their lives. Your primary focus is to educate patients and families about the disease process, manage and control pain and other symptoms, and offer solace to those in distress. Care delivery takes place primarily in the patient's home or in the facility where they receive care.
This position provides valley-wide coverage throughout Maricopa County, requiring you to travel to various locations. You will work within a supportive environment that values your professional skills and your ability to make a tangible difference in the quality of life for those we serve.

Requirements

Nice to Have
Previous hospice or oncology experience. Knowledge of the death/dying and bereavement process. CHLPN certification.
