We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our Hospice team. This role offers a meaningful way to make an impact while enjoying the flexibility of community-based care. As a Hospice LPN, you will deliver personalized, one-on-one care to patients in their homes, working closely with a multidisciplinary team to manage care plans and ensure every patient receives the skilled support they deserve.
Your day-to-day involves providing direct nursing care, including treatments and wound care, while monitoring patient conditions and educating patients and caregivers.
