As an Admission Coordinator, you will play a key role in helping patients and families access compassionate hospice care when they need it most. This office-based position manages the full admissions process, collaborating with referral sources, physicians, case managers, and families to ensure each patient's transition to hospice care is handled with urgency, accuracy, and empathy.
You will serve as the primary contact for new referrals, coordinating all aspects of patient admission within our 3-hour standard. Your day involves communicating promptly with patients, families, and internal clinical staff to ensure a seamless experience. You will verify insurance information, assist billing staff for accurate reimbursement, and maintain accurate data in our information systems. Additionally, you will participate in marketing initiatives and community education events to strengthen relationships with referral sources, while supporting after-hours and weekend admissions as needed to meet patient and family needs.

Experience in healthcare admissions, hospice, or medical office administration. LPN/LVN certification. Bachelor's degree in business, social work, psychology, or nursing. Knowledge of hospice services, Medicare guidelines, and community resources.
