Key Responsibilities
- check_circleMonitor patient progress and report health changes to supervising registered nurses
- check_circleDeliver personalized nursing care by evaluating evolving patient needs and adjusting care plans
- check_circleMaintain accurate and up-to-date electronic medical records using technology systems
- check_circleCoordinate supplies and ensure continuity of patient care with empathy and agility
- check_circleCoordinate care within the interdisciplinary team to ensure effective and efficient delivery
- check_circleTravel to patients' homes or facilities to manage weekly caseloads and provide care
- check_circleProvide education and support to patients and families regarding end-of-life treatment
Requirements
- verifiedLicense and current registration to practice as an LPN in New York State
- verifiedHigh School Diploma or completion of LPN Program
- verifiedValid driver's license (may be required)
- verifiedMinimum of one year work experience as an LPN (preferred)
- verifiedBilingual skills (may be required)
- verifiedHome care/hospice experience (preferred)
Nice to Have
Minimum of one year work experience as an LPN. Home care/hospice experience.
