This role is dedicated to the clinical management of assigned Primary Practice Models (PPM) that serve patients across St. Lawrence County. The position operates within a collaborative dyad relationship with Practice Managers and Coordinators, providing supervision and support to clinical care coordinators, LPNs, and MAs.
As a key partner to the SLH leadership team—including the Directors of Quality, Patient Safety, and Patient Experience—the Clinic Nurse Manager ensures the delivery of high-quality, patient-centered care. The role is instrumental in standardizing infusion practices across the SLH system and assisting inpatient nursing leadership with the planning and operational duties surrounding the administration of chemotherapeutic and biological agents.
You will act as the primary resource for achieving compliance with accreditation standards from Det Norske Veritas (DNV), CMS, and the New York State Department of Health. Your day involves conducting regular clinical practice rounding to focus on quality and audit readiness, while simultaneously fostering a culture of safety. You will oversee the development and maintenance of clinical policies, procedures, and protocols, ensuring they reflect the latest standards of care.
Beyond compliance, you will drive innovation by developing clinical healthcare programs such as Chronic Care Management and Transitional Care Services. You will serve as a Super User for the Electronic Medical Record (EMR), supporting workflow improvements and troubleshooting for staff and providers. Additionally, you will play a vital role in financial stewardship by collaborating with the VP of PPM on budget preparation and the management of medical equipment and supplies.
Candidates are invited to apply for this full-time, 40-hour weekly position. The interview process will evaluate your clinical expertise, leadership capabilities, and alignment with the SLH mission and core values. Successful candidates will be expected to demonstrate strong organizational skills and the ability to navigate complex healthcare environments.
