Clinical Social Worker

About the Role

The Clinical Social Worker is dedicated to maximizing patient well-being and improving outcomes within the dialysis care continuum. This role serves as a vital component of the Interdisciplinary Team (IDT), collaborating closely with doctors, therapists, and medical professionals to ensure comprehensive patient care. The position focuses on helping patients adjust to dialysis through personalized assessments and care plans, while actively supporting the organization's Quality Assurance and Performance Improvement (QAPI) goals.

Day in the Life

You will navigate a fast-paced healthcare environment, balancing independent initiative with high-level collaboration. Your day involves conducting patient assessments, devising care strategies, and managing home support logistics in conjunction with the IDT. A significant portion of your time is spent communicating patient issues and intervention goals to the broader team, consulting with medical staff, and coordinating with admissions to resolve insurance matters. You will also be responsible for data submission via Crown Web and participating in various projects aimed at elevating clinical outcomes.

Work Environment

This position operates within a healthcare facility, such as a dialysis center or a patient's home. The environment features air temperature control and moderate noise levels, though it may involve exposure to infectious materials. The role requires interaction with prospective and current clients, desk work, and travel between assigned facilities for scheduled staff meetings and patient visits.
