This position serves as a Licensed Practical Nurse (LPN) responsible for delivering direct and indirect nursing care to patients within their homes. Working under the supervision of a Registered Nurse (RN), you will collaborate closely with the patient, their family, and the broader healthcare team to implement and evaluate personalized nursing care plans.
Your day will involve conducting comprehensive assessments, performing essential clinical procedures, and monitoring patient conditions to ensure safety and well-being. You will be the eyes and ears of the care team, identifying changes in patient status and communicating effectively with RNs, administrative staff, and physicians to coordinate seamless care. From managing complex wound care and IV therapies to providing patient education and emotional support, you will play a vital role in maintaining the physical and psychological health of those we serve.
