This position serves as a dedicated triage nurse, acting as a vital bridge between patients, families, and a collaborative team of physicians, fellows, and nurse practitioners. The primary mission is to provide comprehensive care coordination, ensuring patients move rapidly and effectively through their episode of care while maintaining the highest standards of quality and safety.
In this role, you will utilize a family-centered approach to assess individualized patient needs and implement evidence-based practices. Your day-to-day activities involve managing complex medical issues through Epic telephone encounters and MyChart interactions. You will communicate test results, coordinate medication refills and prior authorizations, and provide essential patient and family education. Additionally, you will serve as a liaison to ancillary services, including dieticians, home care agencies, insurance companies, and pharmacies, to ensure seamless care delivery.
