About the Role
Caregiver, Inc. is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide direct nursing care and support to an assigned caseload of consumers. This role is vital for ensuring that individuals receiving Wellness Coordination Services have their health needs met with dignity and compliance. Working under the direction of an RN or Clinical Services Manager, you will serve as a key liaison between consumers, their families, and the broader healthcare team.
In this position, you will not only administer care and medications but also act as a point of contact during hospitalizations and assist in training staff on health monitoring and emergency competencies. Your work ensures that consumers receive individualized support in accordance with care plans while maintaining accurate medical records in compliance with HIPAA and state regulations.

Key Responsibilities
- check_circleProvide direct and individualized nursing care to assigned consumers in accordance with care plans
- check_circleDeliver Wellness Coordination Services including required documentation and service provision
- check_circleMonitor and document vital signs, weight, immunization records, and overall health status
- check_circleComplete nursing assessments and documentation per agency policy and LPN scope of practice
- check_circleMaintain accurate consumer medical records in compliance with HIPAA and federal regulations
- check_circleReport changes in consumer health status to the supervising RN or clinical leadership
- check_circleParticipate in Area Individual Support Team meetings and communicate health information
- check_circleAdminister and read TB skin tests for consumers and staff as permitted by state regulations
- check_circleServe as a liaison with healthcare providers and allied service professionals
- check_circleParticipate in the nursing on-call rotation as scheduled
- check_circleAdminister prescribed medications and treatments as ordered in the home setting
- check_circleAssist with staff training on health monitoring, medication administration, and emergency procedures
Requirements
- verifiedCurrent Licensed Practical Nurse (LPN) license in good standing
- verifiedKnowledge of basic nursing care, medication administration, and documentation standards
