About the Role
The Licensed Practical Nurse (LPN) serves as a vital member of the care team, reporting directly to the Wellness Director. This role is designed to monitor the health and wellbeing of residents while providing leadership support and assistance with resident care services.
In this position, you will play a key part in the assessment and Personalized Service Plan (PSP) process, identifying ongoing needs and ensuring proper documentation for any changes in clinical care. You will assist the Wellness Director in completing clinical sections of monthly wellness visits and PSPs to accurately reflect resident status. Additionally, you will be responsible for obtaining weights and vital signs, communicating with resident support services and families, and informing leadership of any changes in medication or service levels that may impact tier levels or residency status.
Shift Details
This position is scheduled for Saturday and Sunday shifts from 10:00 PM to 6:00 AM. The role requires the ability to work weekends, evenings, and flexible hours to ensure availability during peak service delivery times.

Requirements
