This position serves as a vital link in delivering high-quality, holistic nursing care within a long-term care facility. The primary mission is to ensure that all residents receive support in strict accordance with professional nursing practice standards and governing health care regulations. As a leader in the nursing team, you will direct both licensed and non-professional staff to provide exceptional direct resident care and essential support functions.
Your day involves a dynamic blend of clinical oversight, compassionate communication, and operational management. You will conduct daily nursing functions, ensuring that all assigned personnel adhere to safety programs and facility policies. A significant portion of your time is dedicated to the resident journey: admitting new residents, managing transfers and discharges, and participating in orientation for families.
You will collaborate closely with health care providers during rounds to review treatment plans and orders. Your role requires constant vigilance in monitoring resident conditions, completing clinical rounds, and resolving issues with residents or their families before escalating significant events to management. Documentation is a cornerstone of your work, from developing and updating care plans to maintaining accurate medical records and reporting changes in resident status to practitioners and families.
Beyond clinical duties, you will coordinate with interdisciplinary teams—including housekeeping and dining services—to ensure a seamless care regimen. You will also play a key role in staff development, assisting with scheduling, performance monitoring, and training to maintain a high standard of care across the department.
Candidates must possess a current and active license to practice as an LPN/LVN in this state. Preference is given to those with prior experience in nursing within a long-term care facility or similar setting.
