This position serves a defined population by collaborating closely with the broader health care team to coordinate appropriate, cost-effective services. The primary mission is to align care with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life.
In this role, you will act as a vital link between patients, families, and the medical system. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will identify barriers to care, facilitate access to community resources, and guide members through complex health-related legal processes. Whether advocating for organizational resources, supporting families through end-of-life issues, or assisting with guardianship paperwork for virtual court proceedings, your work ensures that care is delivered efficiently and in compliance with ethical standards.
Candidates must hold credentials from the primary work state at the time of hire, with additional state credentials required within six months. The role operates on a rotating schedule (Week 1: Thursday, Saturday; Week 2: Sunday, Friday) and focuses on inpatient settings where applicable.
