This position serves a defined population by collaborating closely with the broader health care team to coordinate appropriate, cost-effective services. The core mission is to align care with each member's plan, helping them achieve their optimal level of independence while enhancing their overall quality of life.
As a key member of the care team, you will manage a caseload by conducting comprehensive psychosocial assessments. These evaluations examine patient goals, social support systems, health status, functional limitations, and environmental factors to ensure resources are utilized efficiently. You will work in tandem with nurse case managers to develop and monitor care plans that promote functioning and well-being.
Your day-to-day impact involves identifying and advocating for organizational and community resources to meet specific care needs. You will facilitate applications for local, state, and federally funded programs (such as Medicaid, Medicare, and Disability) and provide supportive counseling to members, families, and caregivers on critical topics including end-of-life issues and Advanced Directives. Additionally, you will guide members through health-related legal processes, complete guardianship paperwork, and assist families with technology to virtually attend court proceedings when necessary.
This role is dedicated to inpatient care settings. The position operates on a schedule that includes every other weekend and holidays to ensure continuous support for patients and their families.
Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience within a health care setting. This role requires credentials from multiple states; primary state credentials are required at the time of hire, with additional credentials for secondary work states required post-hire. Specific licensing requirements include:
Proficiency in Windows-based computer software, knowledge of community systems in the service area, and familiarity with regulatory issues in the Mid-Atlantic region are preferred.
You will be expected to maintain high social work standards as defined by the NASW Code of Ethics. This includes staying current with case management standards, recent innovations in patient care, and relevant laws and regulations in the local market. Consistent and reliable documentation of all case management activities is essential to comply with organization and department standards.
