This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life. This role is eligible for a $7,500 sign-on bonus.
You will serve as a vital link between patients, families, and the broader healthcare system. Your work involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations, ensuring that medical services are utilized appropriately. You will advocate for organizational and community resources to meet care plans, facilitating access to local, state, and federally funded programs such as Medicaid and Medicare.
A significant portion of your time will be spent providing supportive counseling and education to members, families, caregivers, and healthcare staff, including guidance on end-of-life issues and Advanced Directives. You will also manage complex administrative tasks, such as scheduling family meetings, completing guardianship paperwork, and assisting patients with technology for virtual court proceedings. In an inpatient setting, you will be responsible for completing Uniform Assessment Instruments (UAIS) forms as required by Virginia Medicaid.
This role operates on a rotating schedule: Week 1 includes Sunday, Monday, Wednesday, Thursday, and Friday; Week 2 includes Monday, Tuesday, Wednesday, Friday, and Saturday. The position requires working every other weekend and holidays, with approximately 15% travel required.
Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience in a healthcare setting. This position requires credentials from multiple states. Credentials from the primary work state are required at the time of hire, with additional credentials from secondary work states required post-hire. Specific licensing requirements include:
