This position is dedicated to enhancing the quality of life for a defined population by collaborating closely with the healthcare team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By focusing on optimal independence and holistic well-being, the role ensures that patients receive comprehensive support throughout their healthcare journey.
You will serve as a vital link between patients, families, and the broader healthcare system. Your day involves conducting psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close collaboration with nurse case managers to develop and monitor care plans that promote optimal functioning.

Note: This role is Inpatient Only and requires completion of Uniform Assessment Instruments (UAIs) for long-term care in Virginia.
Candidates must possess credentials from the primary work state at the time of hire. Additional credentials for secondary work states (District of Columbia, Maryland, Virginia) are required within six months of employment. The role is eligible for a $7,500 sign-on bonus.
