About the Role
This position is dedicated to enhancing the quality of life for a defined population by collaborating closely with the healthcare team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By focusing on optimal independence and holistic well-being, the role ensures that patients receive comprehensive support throughout their healthcare journey.
Day in the Life
You will serve as a vital link between patients, families, and the broader healthcare system. Your day involves conducting psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close collaboration with nurse case managers to develop and monitor care plans that promote optimal functioning.
Key aspects of your daily work include:
- Advocating for organizational and community resources to meet specific care needs in a timely and efficient manner.
- Providing supportive counseling and education to members, families, caregivers, and healthcare staff, including guidance on end-of-life issues and advanced directives.
- Navigating complex legal and administrative processes, such as facilitating applications for Medicaid, Medicare, and disability programs, or assisting with guardianship paperwork and virtual court proceedings.
- Maintaining rigorous documentation standards and analyzing program outcomes to drive improvements in quality and cost-effectiveness.
Note: This role is Inpatient Only and requires completion of Uniform Assessment Instruments (UAIs) for long-term care in Virginia.
Hiring Process & Application
Candidates must possess credentials from the primary work state at the time of hire. Additional credentials for secondary work states (District of Columbia, Maryland, Virginia) are required within six months of employment. The role is eligible for a $7,500 sign-on bonus.

Requirements
- verifiedMasters degree in social work (MSW) required
- verifiedMinimum one (1) years of clinical social work experience in a health care setting required
- verifiedLicensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire
- verifiedLicensed Master Social Worker (Maryland) within 6 months of hire
- verifiedLicensed Master's Social Worker (Virginia) within 6 months of hire
Nice to Have
Experience with computer software programs in a Windows environment. Knowledge of community systems and resources in the defined service area. Knowledge of regulatory issues for the Mid-Atlantic area.
