This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life.
As a key member of the care team, you will manage a caseload that requires comprehensive psychosocial assessments. These evaluations consider patient goals, social support systems, resources, health status, functional limitations, psychological status, and environmental factors. Your work ensures that medical services are utilized appropriately and that care plans are designed to promote the member's optimal functioning.
Your day-to-day involves identifying and advocating for organizational and community resources to meet care needs efficiently. You will facilitate applications for local, state, and federally funded programs (such as Medicaid, Medicare, and Disability) and provide supportive counseling to members, families, and caregivers. This includes navigating complex legal processes, such as guardianship paperwork and court proceedings, and offering education on end-of-life issues and Advanced Directives.
This role is designated for Inpatient settings only. The position operates on a schedule that includes every other weekend and holiday coverage. You will be responsible for completing Uniform Assessment Instruments (UAIs) as required by Virginia Medicaid regulations for long-term care.
Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience within a healthcare setting.
This role requires credentials from multiple states. Credentials from the primary work state are required at the time of hire, with additional credentials from secondary work states required post-hire. Specific licensure requirements include:
We expect you to maintain high social work standards as defined by the NASW Code of Ethics. This includes staying current with laws, regulations, and policies relating to social work practice in the local market and local agencies. You will also be expected to promote self-awareness and knowledge of recent innovations in patient care and community case management standards.
