This position is dedicated to supporting a defined population by collaborating closely with the healthcare team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. The role focuses on helping individuals achieve their optimal level of independence while enhancing their overall quality of life.
In this capacity, you will serve as a vital advocate and resource navigator. Your day-to-day work involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will guide members through complex health-related legal processes, including guardianship paperwork and virtual court proceedings, while ensuring consistent documentation of all case management activities.
The role requires close collaboration with nurse case managers and other professionals to develop and monitor care plans that promote optimal functioning. You will also provide supportive counseling and education to members, families, and caregivers, addressing critical topics such as end-of-life issues and Advanced Directives. Additionally, you will facilitate applications for local, state, and federally funded programs and refer cases of suspected abuse or neglect to appropriate community agencies.
Note: This specific role is designated for Inpatient settings only and includes the completion of Uniform Assessment Instruments (UAIS) forms as required by Virginia Medicaid.
This position operates on a rotating schedule: Week 1 includes Sunday, Monday, Tuesday, Thursday, and Friday; Week 2 includes Monday, Tuesday, Wednesday, Friday, and Saturday. The role requires working every other weekend and holidays. Credentials from the primary work state are required at hire, with additional credentials from secondary states required post-hire.
Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience in a healthcare setting. Specific licensure requirements include Licensed Independent Social Worker (DC), Licensed Graduate Social Worker (DC), Licensed Master Social Worker (Maryland), or Licensed Master's Social Worker (Virginia), with the ability to obtain the necessary credentials within six months of hire.
A $7,500 sign-on bonus is available for eligible candidates.
