This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life.
In this role, you will serve as a vital advocate and resource navigator. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close partnership with nurse case managers to develop and monitor care plans that promote patient functioning. Beyond clinical assessment, you will guide members through complex health-related legal processes, facilitate access to local, state, and federally funded programs (such as Medicaid and Medicare), and provide supportive counseling to members, families, and caregivers on critical topics including end-of-life issues and Advanced Directives.
For inpatient settings, the role includes specific responsibilities such as completing Uniform Assessment Instruments (UAIs) to meet Virginia Medicaid requirements. You will also coordinate family meetings and assist patients and families with guardianship paperwork and technology for virtual court proceedings.
Candidates must hold credentials from the primary work state at the time of hire. Additional credentials for secondary work states are required within six months of employment. The role is eligible for a $7,500 sign-on bonus.
