This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life.
As a key member of the care team, you will facilitate the journey from problem identification to the implementation of comprehensive solutions. Your day-to-day involves conducting psychosocial assessments to evaluate patient goals, social support systems, and functional limitations, ensuring that medical services are utilized appropriately. You will work in close partnership with nurse case managers to develop and monitor care plans that promote patient functioning.
Beyond clinical coordination, this role serves as an advocate and educator. You will guide members through health-related legal processes, assist with guardianship paperwork, and provide supportive counseling to families and caregivers on critical topics such as end-of-life issues and Advanced Directives. The role also requires navigating complex funding landscapes, facilitating applications for local, state, and federally funded programs like Medicaid and Medicare, and referring cases of suspected abuse or neglect to appropriate community agencies.

To succeed in this position, candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience within a healthcare setting.
This position requires credentials from multiple states. Candidates must hold credentials from their primary work state at the time of hire. Additional credentials for secondary work states must be obtained within six months of hire:
