This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align care with each member's plan, helping them achieve their optimal level of independence while enhancing their overall quality of life.
In this role, you will serve as a vital advocate and navigator. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close partnership with nurse case managers to develop and monitor care plans that promote patient functioning. Beyond clinical assessment, you will guide members through health-related legal processes, facilitate access to local, state, and federally funded programs (such as Medicaid and Medicare), and provide supportive counseling to members, families, and caregivers on topics ranging from daily care to end-of-life issues and Advanced Directives.
This is an inpatient-focused position requiring the completion of Uniform Assessment Instruments (UAIs) for long-term care, specifically adhering to Virginia Medicaid requirements. You will also coordinate family meetings and assist with guardianship paperwork, including providing technology support for virtual court proceedings.
Please note that this role operates on an every-other-weekend and holiday schedule.
This role is eligible for a $10,000 sign-on bonus.
Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience in a healthcare setting. This position requires credentials from multiple states. Credentials from the primary work state are required at the time of hire, with additional credentials from secondary work states required within six months of employment.
