This position is dedicated to supporting a defined population by collaborating closely with the broader health care team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. The role exists to help individuals achieve their optimal level of independence while significantly enhancing their quality of life.
In this capacity, you will serve as a vital advocate and navigator. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close partnership with nurse case managers to develop and monitor care plans that promote optimal functioning. A key part of your responsibility is identifying and facilitating access to organizational and community resources, ensuring they are implemented efficiently and in a timely manner.
The role also encompasses providing supportive counseling and education to members, families, caregivers, and the health care team. This includes guiding individuals through complex health-related legal processes, assisting with guardianship paperwork, and supporting virtual court proceedings. You will also facilitate applications for local, state, and federally funded programs such as Medicaid and Medicare, and refer cases of suspected abuse or neglect to appropriate agencies.
This is an inpatient-only role. You will be responsible for completing Uniform Assessment Instruments (UAIS) forms, which are a specific requirement for long-term care under Virginia Medicaid regulations. The position operates on a schedule that includes every other weekend and holidays.
Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience within a health care setting. This role requires credentials from multiple states. Credentials from the primary work state are required at the time of hire, with additional credentials from secondary work states required within six months of employment. We expect you to maintain high social work standards as defined by the NASW Code of Ethics. You will be responsible for staying current with laws, regulations, and policies relating to social work practice in the local market, as well as recent innovations in patient care. Strong knowledge of community systems, resources, and regulatory issues in the Mid-Atlantic area is highly valued.
