LCSW-Case Manager, Social Work- In Patient III

About the Role

This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life.

In this role, you will serve as a vital advocate and resource navigator. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close partnership with nurse case managers to develop and monitor care plans that promote patient functioning. A significant portion of the role focuses on identifying and facilitating access to organizational and community resources, ensuring they are implemented efficiently and in a timely manner.

You will also guide members through complex health-related legal processes, including the completion of guardianship paperwork and providing technology assistance for virtual court proceedings. The role requires providing supportive counseling and education to members, families, caregivers, and the broader healthcare team, covering topics ranging from end-of-life issues to Advanced Directives. Additionally, you will facilitate applications for local, state, and federally funded programs such as Medicaid, Medicare, and Disability services.

Work Environment & Schedule

This is an inpatient-only position with a rotating schedule that includes holidays and weekends. The work pattern alternates weekly: Week 1 includes Sunday, Monday, Tuesday, Thursday, and Friday; Week 2 includes Monday, Tuesday, Wednesday, Friday, and Saturday.

Qualifications & Licensure

Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience within a healthcare setting. This role requires credentials from multiple states. Credentials from the primary work state are required at the time of hire, with additional credentials from secondary work states required post-hire. Specific licensure requirements include:

• Independent Clinical Social Worker License (District of Columbia) within 6 months of hire.
• Licensed Certified Social Worker (Maryland) within 6 months of hire.
• Licensed Clinical Social Worker (Virginia) within 6 months of hire.
