This position is dedicated to supporting members of a defined population by collaborating closely with the broader health care team. The core mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By doing so, the role aims to help individuals achieve their optimal level of independence and significantly enhance their quality of life.
In this capacity, you will serve as a vital advocate and navigator. Your day-to-day work involves identifying complex problems, conducting comprehensive psychosocial assessments, and providing financial counseling or referrals. You will guide members through health-related legal processes, connect them with essential community resources, and assist with care placement. A significant portion of your time will be spent collaborating with nurse case managers and other professionals to develop and monitor care plans that promote functional independence.
The role also encompasses supportive counseling and education for members, families, caregivers, and the health care team. This includes addressing sensitive topics such as end-of-life issues and Advanced Directives. You will facilitate family meetings, complete necessary guardianship paperwork, and provide technology assistance to ensure patients and families can virtually attend court proceedings when required. Additionally, you will analyze patient and program outcomes to drive improvements in quality and cost-effectiveness while maintaining rigorous documentation standards.
