The core mission is to collaborate closely with the healthcare team to coordinate appropriate, cost-effective services that align with each member's plan of care. By facilitating access to resources and guiding individuals through complex health-related processes, the role aims to help members achieve their optimal level of independence.
In this capacity, you will serve as a vital advocate, bridging the gap between clinical needs and community support. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in tandem with nurse case managers to develop and monitor care plans that promote functioning while reducing inappropriate medical utilization. From providing supportive counseling on end-of-life issues to navigating guardianship paperwork and virtual court proceedings, this role requires a blend of clinical expertise, resourcefulness, and compassionate communication.
