This position is designed for a dedicated clinical social worker committed to improving the quality of life for a defined patient population. The role serves as a vital bridge between patients, their families, and the broader healthcare team, ensuring that care coordination is both appropriate and cost-effective. By facilitating access to essential services, the social worker helps patients achieve their optimal level of independence while navigating complex health-related challenges.
In this capacity, you will conduct comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. Your work involves identifying barriers to care, advocating for necessary resources, and guiding patients through legal processes, including guardianship paperwork and virtual court proceedings. You will collaborate closely with nurses and other professionals to develop and monitor individualized plans of care, ensuring that every intervention promotes patient well-being and reduces inappropriate medical utilization.
The role also encompasses providing supportive counseling and education to patients, families, and caregivers on critical topics such as end-of-life issues and advanced directives. You will play a key role in analyzing program outcomes to drive improvements in quality and cost-effectiveness, while maintaining strict adherence to the NASW Code of Ethics and local regulatory standards.
Candidates selected for this role will be eligible for a $7,500 sign-on bonus. The position requires credentials from multiple states; primary work state credentials must be held at the time of hire, with additional credentials from secondary states required within six months.
