About the Role
This position is dedicated to improving the quality of life for a defined population by collaborating closely with the health care team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By focusing on psychosocial assessments, resource access, and advocacy, the role aims to help members achieve their optimal level of independence while ensuring efficient utilization of medical services.
You will serve as a vital link between patients, families, and the broader health care system. Your day-to-day involves guiding members through complex health-related legal processes, providing supportive counseling on end-of-life issues and advanced directives, and advocating for necessary community resources. Whether coordinating family meetings, assisting with guardianship paperwork, or supporting virtual court proceedings, you will ensure that care is delivered in a timely, efficient, and compassionate manner.
Note: This role includes a $7,500 sign-on bonus.

Key Responsibilities
- check_circleEnsure consistent documentation of case management activities in compliance with organizational standards
- check_circleAnalyze patient and program outcomes to identify improvements in quality and cost effectiveness
- check_circleComplete guardianship paperwork and provide technology assistance for virtual court proceedings
- check_circleComplete Uniform Assessment Instruments (UAIs) for long-term care in compliance with Virginia Medicaid requirements
- check_circleIdentify and advocate for appropriate community resources to meet patient needs in a cost-effective manner
- check_circleConduct comprehensive psychosocial assessments to evaluate patient goals, resources, and functional limitations
- check_circleDevelop and monitor individualized plans of care to promote optimal patient functioning and quality of life
- check_circleFacilitate applications for local, state, and federally funded programs such as Medicaid and Medicare
- check_circleProvide supportive counseling and education to patients, families, caregivers, and health care professionals
- check_circleSchedule and coordinate family meetings as needed to support patient care planning
- check_circleCollaborate with health care teams to coordinate cost-effective services consistent with patient plans of care
Requirements
- verifiedMasters degree in social work (MSW) required
- verifiedMinimum one (1) years of clinical social work experience in a health care setting required
- verifiedLicensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire
- verifiedLicensed Master Social Worker (Maryland) within 6 months of hire
- verifiedLicensed Master's Social Worker (Virginia) within 6 months of hire
Nice to Have
Experience with computer software programs in a Windows environment. Knowledge of community systems and resources in the defined service area. Knowledge of regulatory issues for the Mid-Atlantic area.
