This position is dedicated to improving the quality of life for a defined population by collaborating closely with the health care team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By focusing on psychosocial assessments, resource access, and advocacy, the role aims to help members achieve their optimal level of independence while ensuring efficient utilization of medical services.
You will serve as a vital link between patients, families, and the broader health care system. Your day-to-day involves guiding members through complex health-related legal processes, providing supportive counseling on end-of-life issues and advanced directives, and advocating for necessary community resources. Whether coordinating family meetings, assisting with guardianship paperwork, or supporting virtual court proceedings, you will ensure that care is delivered in a timely, efficient, and compassionate manner.
Note: This role includes a $7,500 sign-on bonus.
