This position is dedicated to collaborating with healthcare teams to coordinate appropriate, cost-effective services for a defined population. The primary mission is to help members achieve their optimal level of independence and enhance their quality of life through comprehensive care management.
You will serve as a vital link between patients, families, and the broader healthcare system. Your day-to-day involves conducting psychosocial assessments, identifying barriers to care, and advocating for necessary resources. From guiding members through complex health-related legal processes to providing supportive counseling on end-of-life issues, you will ensure that care plans are implemented efficiently and in compliance with ethical standards.
This role is specifically focused on inpatient settings, requiring the completion of Uniform Assessment Instruments (UAIS) forms to meet Virginia Medicaid requirements. You will also coordinate family meetings and assist patients and families with guardianship paperwork and virtual court proceedings.
Candidates must hold credentials from the primary work state at the time of hire. Additional credentials for secondary work states (District of Columbia, Maryland, Virginia) are required within six months of employment.
