This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life.
In this role, you will serve as a vital advocate and resource navigator. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close partnership with nurse case managers to develop and monitor care plans that promote patient functioning. A significant portion of your time will be spent identifying and facilitating access to organizational and community resources, ensuring they are implemented efficiently and in a timely manner.
You will also provide supportive counseling and education to members, families, caregivers, and the broader healthcare community. This includes guiding patients through health-related legal processes, assisting with guardianship paperwork, and offering technology support for virtual court proceedings. The role requires maintaining high social work standards as defined by the NASW Code of Ethics and staying current with laws, regulations, and innovations in patient care.
Note: This is an inpatient-only position requiring the completion of Uniform Assessment Instruments (UAIs) for long-term care, specifically for Virginia Medicaid requirements.
This role operates on a schedule of every other weekend, with alternating holidays.
This role is eligible for a $7,500 sign-on bonus.
Candidates must hold a Master's degree in Social Work (MSW) and possess a minimum of one year of clinical social work experience in a healthcare setting.
This position requires credentials from multiple states. Credentials from the primary work state are required at the time of hire, with additional credentials from secondary work states required post-hire. Specific licensing requirements include:
